Services may be covered in full or in part by your health insurance or employee benefit plan. We ask that you present your insurance card with every visit and we will check your insurance coverage, co-pay, and other relevant benefits. We take most types of insurance including Medicare and Medicaid.

Many insurance policies require co-pay at the time of services rendered. Family Health Partners will file insurance on your behalf and may invoice you for any remaining balance not covered by insurance.

It is helpful for patients to provide us with current insurance plan information prior to services rendered in order for accurate billing of services to be filed.

While our office is enrolled with many managed care plans; we are not able to enroll in all of them. It is the patients responsibility to be familiar with the guidelines of their insurance plan requirements regarding authorizations, deductibles, co-payments and other vital requirements.


Cash, check and most major credit cards are accepted for payment. All co-pays and cash pay visits are due at the time of service.
